Refer A Patient



We appreciate the opportunity to care for your patient.

If this is an emergency or urgent referral, do not follow the steps outlined below. Instead, call our office ASAP at 719-598-0752 to speak with a triage technician who can assess the situation with one of our physicians and schedule the patient appropriately based on the urgency of the condition.

To help us best meet the needs of your patient, please read the exam descriptions below and click on the appropriate link based on the type of evaluation your patient needs.


A medical exam is billed to medical insurance and is for patients with any eye disease or condition other than refractive errors. Common examples include cataracts, diabetic eye exams, age-related macular degeneration, flashes/floaters, double vision, and dry eye. Please note that if your patient is being referred for either glaucoma or a corneal disease, refer to the instructions and referral form at the bottom of this page to schedule with one of our specialists for these conditions. A vision exam is billed to vision insurance (NOT medical insurance) and is for patients with refractive errors (nearsightedness, farsightedness, astigmatism, presbyopia) who need a prescription for glasses and/or contact lenses. A vision exam is not appropriate for eye diseases and conditions other than refractive errors. If your patient has vision insurance and needs a vision exam, please click here to schedule an appointment for your patient.


To refer a patient for either glaucoma or a corneal disease (e.g., keratoconus, Fuchs’ dystrophy, pterygium, history of cornea transplant, etc.), please collect copies of recent exam note(s), visual fields, OCT images, etc. and click here to complete your referral to one of our specialists for these conditions. Following submission of this information, we will contact the patient directly within two business days to schedule an appointment.
